As a condition of participation in the Lake Havasu Music Brews and BBQ AZ. State Championship The undersigned agrees to the following  
1. This event shall take place at the following location: Lake Havasu State Beach State Park Windsor #4, 699 London Bridge Road, Lake Havasu City, AZ. 86403. 2. Lake Havasu Music Brews & BBQ Championship is promoted by C & M Events, LLC./Cooking 4 Kids & Hunger and is a Charity Fundraising Event. Proceeds will benefit our local Food Banks & Education & Recreation intended for Youth & Senior Organization & Charities. Vendors package and confirmations will be sent out via e-mail, C & M Events, LLC. takes no responsibility for vendors not receiving packages or confirmations due to e-mail problems and does not make refunds. 3. This event is based on a first come basis. However C & M Events, LLC. reserves the final right of refusal to any and all craft vendors. Vendor applications will only be considered if completed in full with a sells list attached. This event may have more than one vendor selling the same items or similar items. Fees are based on spaces sized 10' x 10' or 10' x 20'. Full Menu includes but is not limited to those who sell food plates, and other side dishes. Specialty Items include such items as Kettle Corn, Lemonade and Ice Cream. Fees are to be paid in full prior to January 15, 2013 and are nonrefundable. All applications should include two (2) separate checks, one for $50.00 cleaning deposit, which will be refunded at the end of the event when your space has been fully cleared. The second check is for your event space fees & power, we require a check for $300.00 or $500.00 plus any additional for electrical. These fees are non-refundable after you have been accepted and approved as a food vendor. Until fees are fully paid your spot is not guaranteed. 4. There is no “Rain Delay” schedule for this event, and if required the event may be canceled in whole or in part by C & M Events, LLC. without prior notice. In the event of cancellation, vendors shall be refunded their booth fees no later than April 1, 2013. In the event of partial cancellation, there shall be no refund. “Cancellation” shall mean that a vendor has no opportunity to make any sales due to C & M Event’s actions, not weather conditions. C & M Events does not guarantee any particular number of attendees or exhibitors during this event. 5. I agree to follow all guidelines of the Mohave County Health Dept. for food service as included in the Guidelines, including setting up and use of a hand-wash stations. No pets permitted in the food booths or cooking areas. 6. All Food Vendors must have liability insurance. Certificate of insurance with $750,000.00 must accompany this application. Booths without CURRENT CERTIFICATE of insurance AT TIME OF EVENT, will be closed and will forfeit all fees. 7. Food vendors will be assigned to the food court. No Food May Be Sold Or Distributed In Any Other Areas Other Than Your Assigned Space. Vendors may sell only those items submitted and approved on original menu list. All vendors must remain set-up for business during all open to the public event hours. All Vendors must vacate the event premises no later than 12:00 pm on Sunday January 27th, 2013. 8. All items sold, represented or distributed are subject to the approval of C & M Events, LLC. No Alcoholic beverages will be sold or distributed by anyone other than C & M Events, LLC. without prior written authorization & approval. No flammable items or hazardous materials will be allowed. Weapons, controlled substances or similar materials will not be permitted on the premises. 9. I will not dump any ashes, grease or gray water anywhere on the event site, or anywhere else except in the designated receptacles provided by C & M Events, LLC. These receptacles are indicated on your contest event site map. 10. Vendors shall continually police their area for safe and sanitary conditions. At the conclusion of the event, each vendor shall be responsible for his/her own trash removal and shall restore the area to an acceptable condition, as inspected by a designee of C & M Events, LLC. Failure to do so will result in your loss of cleaning deposit. I will remove my own trash from within my booth, and dump in the nearest dumpster located on the event site. Lake Havasu Food Vendor Agreement & Information As a condition of participation in the Lake Havasu Music Brews & BBQ AZ. State Championship The undersigned agrees to the following 11. The event site will be open Thursday January 24th, 12:00 noon - 9:00 p.m. for early set-up and sells starting at 5 p.m. Electrical powder will not be available until Thursday evening and will be shut down Sunday morning January 27th. Vendors with larger rigs are recommended to arrive early. No diesel generators will be allowed for personal use. 12. Assigned vending hours are approximately: Thursday 5:00 p.m. – 9:00 p.m., Friday 11:00 a.m. – 10:00 p.m., Saturday 9:00 a.m. – 10:00 p.m., & Sunday 9:00 a.m. – 2:00 p.m. 13. COMPLIANCE With LAWS: Each vendor is responsible for researching and complying with any and all permits, fees, licenses and any other requirements as outlined by the Arizona State Parks, City of Lake Havasu, Mohave County Environmental Health Department and all other city, county, state and federal agencies. Vendor is also responsible for paying the Arizona State Sales Tax and reporting the location of receipts as Mohave County. In performing the services required hereunder, the performer shall comply with all applicable city, county, state and federal ordinances and codes. 14. HOLD HARMLESS: In recognition that there may be some risk to injury or property damage, in such an undertaking at the Havasu Music, Brews & BBQ State Championship, the undersigned vendor releases and hold harmless C & M Events LLC./Cooking 4 Kids & Hunger and any and all of its members or officers, including Arizona State Parks and any of its representatives for any and all liability for injury or damages to the vendor, or which may result from his/her participation, or which may occur to third parties by the act(s) of the vendors, his/her employees or agents. No action will be asserted or maintained against C & M Events LLC./Cooking 4 Kids & Hunger and any or all of its members or officers and or sponsors including but not limited to Arizona State Parks as a result of any occurrence in any way with the Lake Havasu Music Brews & BBQ State Championship. Further the Vendor shall defend any action so asserted or maintained arising out of any action taken by the vendor, by his or her employees or agents. 15. LIABILITY: As between the parties, each party shall be solely responsible for any and all liability arising from Personal Injury, including death, or damage to property arising from the act or failure to act of the Respective Party or of its officials, Agents and Employees pursuant to this agreement. VENDOR APPLICATIONS WILL NOT BE CONSIDERED UNTIL FULLY COMPLETED & SUBMITTED WITH FEES
